NAVIGATOR PROGRAM

The Greater Trail Hospice Society provides this special program to journey with those who have been diagnosed with life-limiting chronic illness, helping the person and their family to navigate through changes and challenges, connect with resources, learn about their conditions, and anticipate future needs. The Volunteers who are a part of this program have undergone additional training and are committed to providing confidential compassionate care through active listening, assisting with navigating the system, and providing resource materials and information. Interested in helping to build compassionate caring communities? Interested in helping make “life more livable” for persons living with declining health due to life-limiting illness? Do you have 2-3 hours a week to connect with someone in your community? Be a Nav-CARE Volunteer Navigator! Nav-CARE is a free, innovative volunteer outreach program designed to support older persons in our community with daily living challenges. Volunteer Navigators receive training before being matched with clients and ongoing mentorship throughout their time with the program. A Nav-CARE volunteer often: •Provides a friendly ear & encouragement to those in need •Helps individuals & families find services or resources that make their lives easier •Works with individuals & families to describe what is most important to them If you are interested in learning more about this rewarding program or to register for the training starting September 13th, contact Linda at 250-364-6204 or trail.navcare@gmail.com.
